KADIAN (MORPHINE SULFATE)

Morphine sulfate is a strong analgesic used to relieve severe, acute pain or moderate to severe, chronic pain (e.g., in terminally ill patients). The drug is also used parenterally for preoperative sedation, as a supplement to anesthesia, and for analgesia during labor. Morphine sulfate is also available as a preservative-free injection (AstramorphÆ PF, DuramorphÆ PF, InfumorphÆ) that can be injected epidurally or intrathecally for relief of severe pain (neuraxial analgesia); administration of the drugby these routes reportedly provides pain relief for prolonged periods without attendant loss of motor, sensory, or sympathetic function.

Fentanyl citrate is a strong analgesic used preoperatively, during surgery, in the immediate postoperative period for its analgesic action. Fentanyl citrate also is used intrabuccally (transmucosally) in the management of breakthrough cancer pain.

The DURAGESICÆ (fentanyl transdermal system)CII patch is a strong medication for moderate-to-severe chronic pain, available only by prescription, that can provide long-lasting relief from persistent pain.

Through its unique patch technology, DURAGESIC delivers fentanyl, an opioid pain medication, slowly into the body through the skin where it works to relieve pain for up to 3 days (72 hours).

As an analgesic, methadone is used in the relief of severe pain. Methadone oral solution is used to relieve severe, chronic pain (e.g., in terminally ill patients). The oral concentrate solution may be used for the relief of severe pain and in detoxification and maintenance treatment. Although the drug has antitussive activity, methadone is no longer approved by the US Food and Drug Administration for this indication.

Hydromorphone is a strong analgesic used in the relief of moderate to severe pain. Hydromorphone hydrochloride injection containing 10 mg of the drug per mL is a highly concentrated form of the drug that should only be used in patients who are tolerant to opiate agonists and are currently receiving high dosages of an opiate agonist (e.g., patients being treated for severe chronic pain associated with cancer). This highly concentrated injection may be particularly useful in patients in whom IM orsubcutaneous injection of large volumes of other opiate agonists is associated with discomfort or is precluded because of small muscle mass (such as in emaciated patients). The drug also has antitussive activity which makes it useful in the control of persistent, nonproductive cough.

It is available in:

Ampules (for parenteral administration) containing:

1 mg, 2 mg, and 4 mg hydromorphone hydrochloride per mL with 0.2% sodium citrate, 0.2% citric acid solution. DILAUDID ampules are sterile.

Multiple Dose Vials (for parenteral administration) containing:

20 mL of solution. Each mL contains 2 mg hydromorphone hydrochloride and 0.5 mg edetate disodium with 1.8 mg methylparaben and 0.2 mg propylparaben as preservatives. Sodium hydroxide or hydrochloric acid is used for pH adjustment. DILAUDID multiple dose vials are sterile.

Color Coded Tablets (for oral administration) containing:

1 mg hydromorphone hydrochloride (green tablet) and FD& C blue #1 Lake dye, DEC yellow #10 Lake dye, lactose, and magnesium stearate. 2 mg hydromorphone hydrochloride (orange tablet) and D8C red #30 Lake dye, D& C yellow #10 Lake dye, lactose, and magnesium stearate. 3 mg hydromorphone hydrochloride (pink tablet) and D& C red #30 Lake dye, lactose, and magnesium stearate. 4 mg hydromorphone hydrochloride (yellow tablet) and D& C yellow #10 Lake dye, lactose, and magnesium stearate.

Suppositories (for rectal administration) containing:

3 mg hydromorphone hydrochloride in a cocoa butter base with silicon dioxide. Non- Sterile Powder (for prescription compounding) containing hydromorphone hydrochloride.

Oxycodone is a mild analgesic used to relieve moderate to moderately severe pain, such as that associated with bursitis, injuries, dislocations, simple fractures, and neuralgia. The drug is also used in the treatment of postoperative, postextractional, and postpartum pain. Pain of an acute nature appears to be more responsive to therapy with oxycodone than is pain of long standing.
